Right to Access Your Records
You have the right to inspect and obtain a copy of your health information that we use to make decisions about your care. We may charge a reasonable fee for copies. We will respond to your request within 30 days.
Right to Request Amendments
If you believe that health information we have about you is incorrect or incomplete, you may request that we amend it. We may deny your request under certain circumstances, and if we do, we will explain why in writing.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your health information. This right applies to disclosures made for purposes other than treatment, payment, or healthcare operations.
Right to Request Restrictions
You have the right to request restrictions on how we use or disclose your health information for treatment, payment, or healthcare operations. We are not required to agree to your request, but if we do, we will comply with it except in emergency situations.
Right to Confidential Communications
You have the right to request that we communicate with you about your health matters in a certain way or at a certain location. We will accommodate all reasonable requests.

You have the right to receive a paper copy of this Notice of Privacy Practices at any time, even if you have agreed to receive it electronically. Contact us using the information below and we will provide one promptly.
Our Responsibilities
Kitchens Home Care is required by law to:
- Maintain the privacy and security of your protected health information.
- Notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
- Follow the duties and privacy practices described in this notice and provide you with a copy of it.
- Not use or share your information other than as described here unless you authorize it in writing.
We reserve the right to change our privacy practices and the terms of this notice at any time, provided such changes are permitted by applicable law. We will make the new notice provisions effective for all health information that we maintain, including information previously created or received before the update. We will post the revised notice on our website and make copies available upon request.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with Kitchens Home Care or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
